Understanding Your 1099-R Form

-In the Mail to You by January 31st

This form lists your 2015 retirement benefit income from MOSERS. You will need this form to file your income tax return. The 1099-R provides the following information for the 2015 calendar year:

1 Total gross distribution (The total amount paid to you by MOSERS for the calendar year.)

2a Total taxable amount (The portion of your total benefit received during the calendar year that should be considered taxable income.)

4 Taxes withheld (The total amount of federal income tax withheld during the calendar year.)

5 Employee contributions/designated Roth contributions (Difference in Box 1 and Box 2a - the portion of service purchase excluded from taxable amount - only applies to after tax funds)

7 Distribution Code(s) (Code identifying the type of benefit being paid - see chart on page 2 of your 1099-R)

9b Total Employee Contributions (Remaining balance of what was paid in for purchase of service to be applied in future years (decreases yearly))

12 State tax withheld (The total amount of Missouri income tax withheld during the calendar year.)

Health Insurance Deductions (The total amount of medical premiums withheld during the calendar year.)

Dental Insurance Deductions (The total amount of dental insurance premiums withheld during the calendar year.)

Vision Insurance Deductions (The total amount of dental insurance premiums withheld during the calendar year.)

Contact your insurance provider for more detailed information on the deductions from your MOSERS benefits. You will also be able to access your 1099-R form on MOSERS’ website after they have been mailed if you need a replacement copy.
